Here’s how the program works

  • Employee Dashboard

    Each employee will be contacted by e-mail and encouraged to complete a free, online Health Assessment that helps determine their current state of health. The information is confidential. It is not shared with the employer or with the insurance carrier.
  • In return for completing the Health Assessment, each employee will receive a $50 electronic gift card.*
  • Employees receive a report that outlines their general state of health and highlights areas for improvement.
  • Employees will be encouraged to regularly visit the interactive CBIA Healthy Connections website as often as they'd like to track their progress, receive educational information, and to participate in wellness workshops.

Major features of the website include:

  • Nutrition Resource Centers - articles and tips to help users create their own strategies
  • Meal Planner and Nutrition Tracker - a personalized planning and tracking tool linked to over 7,000 foods from the FDA, with complete nutritional info
  • Exercise Tracker - enables users to know how many calories they're burning
  • Weight Scale - lets users weigh in to track progress
