    Employees can choose from five benefit plans, electing the benefit that best fits their needs. There’s even a plan with orthodontia coverage for dependents up to age 19.
  • Dental Rewards: The Passive PPO 100%/80%/50%-$1,000 and the Passive PPO 100%/80%/50%-$1,500 with Ortho dental plans include a valuable feature that allows qualifying plan members to carryover part of their unused annual maximum. Learn more here.

Ameritas Online Access for Enrolled Members

When you create a secure member account, you have access to ID cards, plan benefits, your certificate of coverage, claim information, and your remaining benefits. You can also sign up to receive your Explanation of Benefits (EOB) electronically.

  1. Go to ameritas.com
  2. Select the "Account Access" link in the upper right corner of the page which will bring up the "Account Access" page.
  3. Select the Dental/Vision/Hearing drop down
  4. Under Member, choose either "Secure Member Account" or "Secure Member Account (NY)" as applicable
  5. On the Login page select "Register Now"
  6. Complete the New User Registration form and select "Create Account"
  7. Select "Continue" on the Account Registered page
  8. Enter your first name, last name, date of birth, and zip code on the User Authorization page and select "Submit"
  9. On the Protected Health Information Access page, enter your Social Security Number or member ID or enter a claim number and source code from a recent claim EOB statement and select "Submit"
