Here are the different medical networks available to employers, dependent on the plan.

FlexPOS Plans

First Health is a national healthcare provider network that supplements the ConnectiCare proprietary network and gives members nationwide coverage.

To find a doctor, go to Find a Doctor and answer a few questions about the type of healthcare professional you need and where you get your health insurance. If searching for a type of provider other than dental, chiropractor, or mental health/substance abuse, select the following options.

• Type of network or plan: FlexPOS
• Show me options for: Doctors or Facilities
• On the next page, you will be prompted to enter the specific details for the name, location, or the type of provider you are searching. This search will include providers and facilities in both the ConnectiCare local area network and the First Health national network.

If you are searching for a provider in any of the New York counties listed below, please select the red Quick Link at the top of the provider search page. This will direct you to the EmblemHealth Prime network directory. EmblemHealth Prime Network counties are: Albany, Bronx, Broome, Columbia, Delaware, Dutchess, Fulton, Greene, Kings, Montgomery, Nassau, New York, Orange, Otsego, Putnam, Queens, Rensselaer, Richmond, Rockland, Saratoga, Schenectady, Schoharie, Suffolk, Sullivan, Ulster, Warren, Washington, Westchester.

Passage Plans

Passage plans promote close relationships between patients and primary care physicians (PCPs). Each enrolled family member must select a PCP at the time of enrollment. Referrals are required to seek care from specialists.

Members considering a Passage plan should confirm that their PCP is a participating Passage PCP or that they are willing to select a participating Passage PCP. Passage PCPs are located in Connecticut only. View the key at the bottom of the Find a Doctor search results page to help identify Passage PCPs. Note: Passage members may also use specialists and facilities in New York using the Emblem Health Prime Network.

Compass Plans

Members pay less in copays, deductibles, and/or coinsurance when they visit preferred PCPs, specialists, and hospitals for covered services.

Use Find a Doctor to locate Compass preferred providers. All Compass preferred providers are located in Connecticut. View the key at the bottom of the Find a Doctor search results page to help identify Compass PCPs. Note: The Emblem Health Prime Network in New York is not available to members with Compass plans.

ConnectiCare Benefits, Inc (CBI)* Plans

CBI plans offer employees additional plan options at competitive price points. CBI plans use a Connecticut-only network and a different formulary than other small group plans. Note: The Emblem Health Prime Network in New York is not available to members in CBI plans.

How to Print Temporary ID Cards

1. Go to ConnectiCare.com and click Sign In.
2. Under Individual and Family Members, click Sign In.
3. If you do not have access, click Register, under New to ConnectiCare? and proceed to step 4. Otherwise, proceed to step 6.
4. Enter first and last name, birthdate, email, and member ID number, then click Continue.
5. Verify your information, then click Submit.
6. Once signed in, click Care Dashboard.
7. Print Temporary ID Card is the third option on the left.
