First Health is a national healthcare provider network that supplements the ConnectiCare proprietary network and gives members nationwide coverage.

To find a doctor, go to Find a Doctor and answer a few questions about the type of healthcare professional you need and where you get your health insurance. If searching for a type of provider other than dental, chiropractor, or mental health/substance abuse, select the following options.

• Type of coverage: Medical
• How do you get insurance: Through my Employer
• Type of network or plan: FlexPOS
• Show me options for: Doctors or Facilities
• On the next page, you will be prompted to enter the specific details for the name, location, or the type of provider you are searching. This search will include providers and facilities in both the ConnectiCare local area network and the First Health national network.

If you are searching for a provider in any of the New York counties listed below, please select the red Quick Link at the top of the provider search page. This will direct you to the EmblemHealth Prime network directory. EmblemHealth Prime Network counties are: Albany, Bronx, Broome, Columbia, Delaware, Dutchess, Fulton, Greene, Kings, Montgomery, Nassau, New York, Orange, Otsego, Putnam, Queens, Rensselaer, Richmond, Rockland, Saratoga, Schenectady, Schoharie, Suffolk, Sullivan, Ulster, Warren, Washington, Westchester.

How to Print Temporary ID Cards

1. Go to ConnectiCare.com and click Sign In.
2. Under Individual and Family Members, click Sign In.
3. If you do not have access, click Register, under New to ConnectiCare? and proceed to step 4. Otherwise, proceed to step 6.
4. Enter first and last name, birthdate, email, and member ID number, then click Continue.
5. Verify your information, then click Submit.
6. Once signed in, click Care Dashboard.
7. Print Temporary ID Card is the third option on the left.
